Planning my way to work 2.0

For youth who experience disabilities, thinking about adult life can be exciting! However, the transition process can be complex, and can frequently involve planning for employment, higher education, career development, and navigating other adult services. In this new edition of the Planning My Way to Work transition guide students, families, teachers, and case managers will be given a variety of tools. The tools are aimed to support meaningful conversations, and develop person centered transition plans, designed to grow, and change with the student. The tools also help to highlight what the person wants and wants to avoid as they progress through their own transition experience.
