1. Please download the 2009 California Charter Schools Conference Exhibit Space Application/Contract and read it carefully.
   
   
2. Print out the Application/Contract, print or type all information requested, sign, and date.
   
   
3. Remit Application/Contract with full payment of total exhibit booth space rental* to:

   1277 University of Oregon
   Eugene, OR 97403-1277
   
   *Make checks payable to the California Charter Schools Association (CCSA).
   
   
4. Confirmation of your booth assignment will be sent to you following receipt of signed Application/Contract and full payment.
